- Comprehensive Exam Template (due no later than 1 week prior to exam date)
- Description of Duties and Allocation of Hours (DDAH) Form (due within 15 days of the letter of offer)
- Dissertation Prospectus Record
- PhD Final Oral Exam (FOE) Booking Request (due no later than 8 weeks prior to exam date)
- Progress Report (due May 31, 2013)
